The forms above are “fillable” PDF files, so you can type all necessary info into the form, then either print the completed form to bring with you to your first visit, or you can click the “Email Form” button at the top of the first page of the form to email it to us – this will open a new message within your default email program (ie Outlook Express, Windows Mail) pre-addressed to us and with the completed form already attached, so you just have to click send (you can add a personal message if you like before sending). Either way, this ensures that when you come for your first visit we can get started right away!
